Private provider evidence stays off-chain. Raw documents, provider records, complaints, and carrier names are not written to Hedera. Only hashes, report hashes, policy versions, and non-sensitive audit-event metadata should be anchored. HCS provides event ordering and consensus timestamps; hash-only anchoring can prove integrity without exposing confidential records.

A production design could map authorized approvers to allowlisted Hedera accounts or cryptographic identities, have each approver sign the exact proposal hash, verify quorum before activation, and anchor proposal, approval, and activation events to HCS. HCS can prove event order and consensus timestamps; it does not by itself prove that a person holds a corporate role.
